This chapter will examine the health literacy interview in Section B of the MDS, Section C of the MDS, and other post-acute care assessment item sets. The information obtained from both of these interviews contributes to care planning, payment, and quality measures.
2. Section D: Mood and Social Isolation
This chapter will examine Section D of the MDS and other post-acute care assessment item sets. Section D assesses mood and contributes to care planning, payment, and quality measures.
3. Section J: Health Conditions
